Prevent Type 2 Diabetes Before It Starts

Trinity Health Michigan Diabetes Prevention Program

Prediabetes means your blood sugar is higher than normal, but not high enough for diabetes. The good news is type 2 diabetes can often be prevented or delayed. Trinity Health Michigan offers the nationally known Diabetes Prevention Program to help you build healthy habits that last.

Program Overview

The Trinity Health Michigan Diabetes Prevention Program is a one‑year lifestyle change program. You work with a trained lifestyle coach and others who share the same goals. The program focuses on small, realistic changes in eating, movement, and daily habits that help lower your risk for type 2 diabetes.

Years of research have shown that this program, part of the CDC National Diabetes Prevention Program, can reduce the risk of developing type 2 diabetes

Who Is Eligible?

You may qualify for the program if all the following are true:

  • You are 18 years or older
  • You are not pregnant
  • You have never had type 1 or type 2 diabetes
  • Your BMI is 25 or higher (23 or higher if you are Asian)

You also need one of the following:

  • A blood test in the past year showing prediabetes
    (A1C 5.7–6.4% or fasting blood sugar 100–125 mg/dL)
  • A history of gestational diabetes
  • A high‑risk score on the CDC’s Prediabetes Risk Test
