We are a primary care practice where board-certified physicians and physicians-in-training offer primary care services for the entire family − from prenatal care to infants and older adults. Our health care team provides exceptional medical care for the entire family.

 

If you have an urgent medical problem and need assistance after business hours, please call the office and your call will be directed to the Trinity Health IHA Call Center. The call center staff will assist you and contact the physician on-call to address your concerns.

Trinity Health Academic Family Medicine - Brighton is a designated Patient-Centered Medical Home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support needed to make decisions and participate in their own health care.
